Gallbladder stones (Cholelithiasis) are solid crystalline concretions formed within the gallbladder lumen. The liver secretes bile containing bile acids, phospholipids, and cholesterol. When the liver synthesizes excessive cholesterol or secretes inadequate bile salts (biliary supersaturation), cholesterol precipitates out of solution as microscopic crystals. Impaired gallbladder motility (hypomotility) allows these crystals to aggregate into biliary sludge and grow into hard gallstones. When a stone temporarily occludes the cystic duct, intense visceral smooth muscle spasms produce acute biliary colic: severe, sharp epigastric or right upper quadrant pain radiating to the right shoulder blade, accompanied by nausea, severe vomiting, and intolerance to fatty foods.
